·         A complete Bill-To Address and Ship-To Address

·         Purchase Order Number

·         A 10 digit ISBN 

·         Quantity of texts to be shipped

·         First Author’s Name and Title of Book

·         The name of the Instructor(s) who will be using the book

·         Preferred Method of Shipment–standard, two-day, or overnight (or your own shipping information if you wish to use your own shipping carrier).
